Please be advised that for the upcoming outreach activity in Malta, POLO shall only be processing verification of contracts of workers with previous record with POEA.

Please submit your names to the Honorary Consulate of the Philippines in Malta for initial verification on or before 26 March 2018. Kindly bring the required documents on the day of Outreach upon confirmation.

REQUIREMENTS FOR ON-SITE REGISTRATION

Renewal of Contract (Same Employer)

  1. Previously verified contract
  2. Newly signed contract
  3. Undertaking
  4. Photocopy of Work Permit
  5. Latest NI Payments
  6. Photocopy of Worker’s Passport
  7. Photocopy of Employer’s ID with Signature

Change of Employer

  1. Salaysay or Affidavit detailing circumstances of new employment
  2. Signed Contract
  3. Undertaking
  4. Photocopy of Work Permit
  5. Latest NI Payments
  6. Photocopy of Worker’s Passport
  7. Supporting documents of new employment:
  8. Employer’s business registration/Employer’s Information Sheet
  9. Photocopy of ID – Employer or Authorized Signatory
  10. Proof of Financial Capability

All documents must be submitted in sets of three (3) – one (1) original and two (2) photocopies.
